Description of Service The Health Access Worker service will improve access to healthcare for people sleeping rough by providing wraparound connections for rough sleeping clients into the health system. The service will provide lived experience as a core component of provision. The Health Access Worker will: Improve access to healthcare for people sleeping rough by providing wraparound connections for rough sleeping clients into the health system. Be headquartered at the Outreach provider’s office, but with the understanding that it is peripatetic/out in the field for a primary portion of the workday. Provide lived experience as core component of service provision. Be expected to work 3 days per week and to meet with clients outside business hours as needed. Improve access to healthcare for people sleeping rough. Increase the number of GP registrations. Increase attendance at health appointments. Reduce missed GP and secondary healthcare appointments. Prevent deterioration of health to reduce escalation to crisis point and minimise risk of long-term impact of rough sleeping on physical health.
